Forms and Permissions.

There are a number of forms we require as part of our enrolment process.

If you live outside the school zone, please access school zone from the same menu as this on the homepage to complete and submit a ballot application form.
Enrolment Form - complete only if you live within the school zone or at the point when you have been offered a place for those out of zone - please complete online, access via this link enrolment.

The remaining information needs to be printed off, completed and either scanned and emailed, or brought into or posted to the school office.

Birth certificate or passport. If not a New Zealand or Australian citizen then proof of permanent residence will be needed.
Motor Vehicle Child Passenger Restraint.
Address statement for all applicants living within the Ladbrooks School Zone.
Digital Use Protocol which includes BYOD and Cyber-safety guidelines and agreement of use.
Immunisation certificate for new entrant aged children. This certificate is in a child's Plunket book and maintained by a family doctor. Please advise if no immunisations have been given to your child so this can be noted.

Enrolment will not be finalised until the above are submitted.
